A Bacteriologic Study of Acid-fast Bacilli Isolated from Pulmonary Tuberculosis


Abstract
Accuracy of monitoring of the pulmonary tuberculosis was assessed by microscopic and cultural results from 146 patients who were admitted to the National Kong Ju Hospital in June 1980.
The sensitivity testing to isonizid, streptomycin and ethambutol was done using the absolute concentration method.
The results were summarized as follows:
1. The sex distribution of 146 patients was 89 males and 57 females.
2. The peak incidence occured in 20 to 29 age group.
3. The incidence of culture positive was 45.8 per cent, in which 11.9 ...per cent culture positive were found only on media containing streptomycin(2pg/ml) or isonizid (0.2iig/ml).
4. Only 64.2 per cent of total culture positive were direct smear positive. The incidence of direct smear positive and culture negative was 4. 1 per cent.
5. Approximately one fifth of the patients might have been misdiagnosed when the direct smear was only used without culture.
6. In drug sensitivity test, drug resistant strains were found 16. 1 per cent in isoniazid (1pg/ml), 35. 5 per cent in streptomycin (10pg/ml) and 40. 3 per cent in ethambutol (2pg/ml ).
Conventional microscopic techniques are significantly less sensitive than culture at detecting the presence of mycobacteria in sputum. Therefore, available culture methods are essential for accurate monitoring of pulmonary tuberculosis.
It is obvious that the prevalance of pulmonary tuberculosis has declined with effective drug regimens in Korea. But the control of tuberculosis still remains a difficult problem to solve. It is mainly caused by the increasing occurence of resistant bacilli which is attributable to inadequate drug medication. The correct chemotherapy based on reliable sensitivity test. It prevents the occurence of resistant bacilli, relapses and stops the progress of the disease.
